What this policy covers

This policy covers choosimedspa.com only: what we collect when you browse, request our panel guide, or fill out a form on this site, and how we use it.

Booking happens on a separate platform. When you click a booking link on this site, you leave this website for Zenoti, our scheduling system, and the information you enter there is handled under Zenoti’s own privacy practices, not this page. Once you are a patient, your medical record is created and kept confidentially, under separate healthcare privacy rules, not the ones described here.

Information we collect

Information you give us

If you request our panel guide or fill out our contact form, we collect only what you type into that form, typically your name and email address. No form on this website asks for, or collects, any health information, symptoms, medications, or medical history. If you have a clinical question, please call our office directly instead of using a web form.

Information collected automatically

Like most websites, this site uses Google Analytics 4 to understand how it is used in aggregate: which pages are viewed, how visitors arrived, and general device and location information, using small text files (cookies) stored in your browser. Analytics is configured with IP anonymization, so your full IP address is not stored, and this data is never used to identify you individually or to build or add to any medical record.

How we use your information

We use what you give us to send the resource you asked for, respond to your message, and, in aggregate and anonymized form, to understand how this site is used so we can improve it. We do not sell your information, and we never use it to build a medical record.

Email and text message consent

Submitting your email address to request the panel guide, or through any other form on this site, is your consent for Choosi Med Spa to send you that resource and related email. You can unsubscribe at any time using the link at the bottom of any email we send.

If a form on this site ever asks for, and you provide, a phone number, submitting it is your consent to receive text messages from us related to your request. Message and data rates may apply, message frequency varies, and you can reply STOP at any time to stop texts, or HELP for help. Declining email or text communication never affects your ability to book or receive care.

Not a medical record

This website does not create a patient relationship

Browsing this site, downloading the panel guide, or submitting a form here does not create a doctor-patient relationship and does not create or add to any medical record. A name or email address you give us through a website form is handled as ordinary business contact information, not as part of your chart.

A clinical relationship begins once you have been seen in person or through a scheduled telehealth visit, and your care has been reviewed by board-certified OB/GYN Dr. Tiffany Hall or our medical team. If you have a symptom, medication, or health question, please call our office at 513.818.8171 rather than sending it through a web form.

Third parties

Who we share information with

We do not sell your personal information. We share it only with the services that keep this website and our booking system running: our email provider, to send what you requested or respond to your message; Google Analytics, in aggregated and anonymized form; and Zenoti, our booking platform, only if you choose to click through and book, and only with the information you enter there directly.

We use reasonable administrative and technical safeguards to protect the information you share with us. No method of storage or transmission over the internet is completely secure, so we cannot guarantee absolute security.
