What is actually happening?

Texture is what you notice when light stops reflecting evenly. Raised and depressed areas across the same square inch of skin cast small shadows, and the eye reads that as roughness before it identifies any single feature. Which is why texture is often described by touch, or by how makeup sits, rather than by anything visible in a mirror at arm's length.

Several things create it and most of them compound. Cell turnover slows with age, so dead cells linger on the surface instead of shedding. Ultraviolet light breaks down collagen and elastin, and skin thickens unevenly in response, which is where rough patches and stretched-looking pores come from. Dehydration compromises the barrier, so the skin sheds less effectively. Past breakouts leave small indentations behind. And aggressive scrubbing at home can add micro-tears that make the surface less even, not more.

The distinction worth making at the first visit is between texture and scarring. Broad, shallow roughness responds to resurfacing. Discrete depressions from old lesions are acne scars and need collagen work at a specific depth.

A non-ablative 1927 nm thulium laser. It creates roughly 12,000 microchannels per pass, which prompts collagen production underneath while the surface renews on its own, and a full-face pass takes 6 to 8 minutes. RF microneedling, for density as well as surface

Fine needles create controlled channels while radiofrequency energy heats the tissue below, which builds collagen and elastin together. Exfoliation as the maintenance layer

How soon does texture change?

Can enlarged pores be made smaller permanently?

Pore size is largely genetic, so the honest framing is that pores can be cleared and their appearance refined rather than removed. Treatments reduce congestion and rebuild the collagen around the opening, which is what makes the surface read as smoother.

Is exfoliating at home enough?

It helps with surface buildup and does nothing for the deeper causes, which are collagen loss and scarring below the surface. Aggressive scrubbing can also create micro-tears that make texture worse, which is a common reason people arrive here after a year of trying.
